After scheduling your appointment and before visiting the Center, you’ll receive a Welcome Package by mail. This package includes a detailed itinerary of your appointment day, a list of tests you will receive, forms you must complete and any special instructions related to your program.

Pre-Visit Questionnaire

Before your appointment at The Center for Preventive Medicine^® North Texas please complete the Health History Questionnaire and accompanying food diary that you receive in your Welcome Package. We ask that you complete and return all paperwork seven days prior to your visit so we have optimal time to review your responses before meeting with you.

Pre-Visit Lab Work

We may ask you to submit blood and urine samples a week or two prior to your arrival to ensure the results can be processed and available for review by your appointment day.

In many cases, we will schedule a time for a phlebotomist to come to your home or workplace at a time convenient for you.  Keep in mind that fasting several hours before this blood work may be required, so please read the all instructions carefully.
