At The Center for Preventive Medicine^®, physicians work in concert with your primary care doctor to ensure you receive the very best care. All testing details and recommendations for lifestyle changes, treatments, testing and procedures are communicated to ensure that all health care professionals involved in your care are on the same page.

Upon completion of your follow-up visit, you and your doctor will complete the required paperwork, sign and date the voucher and enclose a copy of his or her consultation notes before returning the information packet to your local Center. After processing this information, your doctor will be reimbursed by the Center.
